Breast lift
A breast lift may be advisable if the breasts have lost their shape, firmness and projection following pregnancy, breastfeeding, weight fluctuations or natural ageing processes. Many patients do not necessarily want larger breasts, but rather breasts that look more harmonious, firmer and more natural.
For Dr Wiedner, a breast lift is not simply a matter of removing skin. The key lies in the internal shaping and stabilisation of the tissue. Only by considering the skin envelope, mammary gland, nipple position and tissue tension as a whole can a long-lasting, natural-looking result be achieved.
Breast reduction
A breast reduction may be performed for both aesthetic and functional reasons. Large, heavy breasts can lead to neck, shoulder or back pain, skin irritation,
restrictions on physical activity or a constant feeling of physical strain.
The aim of a modern breast reduction is therefore not simply to reduce volume. It is about relieving pressure, achieving better proportions, a more natural breast shape and long-term tissue stability. Dr Wiedner plans the reduction so that the breast not only appears smaller, but also more harmonious, lighter and better integrated into the overall body silhouette.

Breast augmentation
For Dr Wiedner, modern breast augmentation does not begin with the question of a specific size. What is far more important is determining which volume, shape and tissue load will suit the patient’s anatomy in the long term.
Modern implants, autologous fat or hybrid procedures may be appropriate, depending on the patient’s initial situation. The focus is not on maximum enlargement, but on achieving a breast that integrates naturally into the body. In particular, tissue quality, skin elasticity, the breast base, the ribcage and natural proportions determine whether the result will appear harmonious in the long term.

Implant removal and breast reshaping
After years of living with implants, many female patients wish to make a change: smaller implants, a more natural breast shape, or complete implant removal. Reasons for this may include capsular contracture, a feeling of tightness, thinning of the tissue, ageing of the implants, feelings of insecurity associated with the implants, or simply a changed body image.
However, implant removal usually involves more than simply taking out an implant. The key factor is how the skin, breast tissue, capsule, volume and surrounding tissue can be restored to a stable and natural shape afterwards. Depending on the findings, a breast lift, autologous fat transfer, internal stabilisation or a phased approach may be appropriate.

Revision breast surgery
Revision breast surgery is one of the most challenging areas of breast surgery. Previous operations can result in scarring, capsular contracture, implant malposition, rippling, bottoming-out, asymmetry, tissue atrophy or unnatural tension.
For Dr Wiedner, breast revision is not simply a matter of performing ‘more’ surgery. Often, the opposite is true: tissue must be allowed to settle, tension reduced and a natural shape restored. A precise analysis of the existing structures and realistic planning to determine which improvements can be achieved in a stable and meaningful way in the long term are crucial.

Which breast treatment is the right one?
The right treatment cannot be determined solely on the basis of a patient’s wishes or a photograph. A breast may have lost volume but may also require a lift. A breast that is too large can cause functional problems. Following implant surgery, the tissue may be stable enough to take on a new shape — or it may initially require rest and gradual reconstruction.
An individual assessment is always crucial:
What is the quality of the tissue?
How resilient is the skin?
What is the current shape of the breast?
Have there been any previous operations or implants?
What role do autologous fat, implants or a lift play?
And which result will feel natural and harmonious in the long term?
A consultation with Dr Wiedner is therefore not intended to recommend a specific method before the anatomy has been understood. It serves to work out together which strategy is best suited to the patient, her body and the long-term stability of the breast.
